Combine and stir all the above ingredients except the egg whites and pineapple. Whip the egg whites until stiff and fold them into the mixture. Place crushed pineapple at the bottom of a buttered ovenproof dish. Pour the custard mixture over the fruit. Place the baking dish on a rack in a pan of hot water. Bake the custard for about 50 min.
